These steps will help expedite the re-certification. Please follow the steps carefully to avoid any delays.
- Download re-certification packet (if you have not already done so) read the instructions and verify you meet all qualifications. To download the packet, click here. This is a PDF file and will open in a new window. Once downloaded print the document then close the window and return to this page.
- Make online payment by clicking here. Again, this will open in a new window. Once the payment process is completed, write down the PayPal receipt number and keep it handy. Close the window and return to this page.
- Complete application form (part of the re-certifcation packet printed in step 1). Complete all required information. In the payment section, indicate payment was made with PayPal and include the receipt number, date of payment and the name of the person making the payment if other than your own.
- Copy of License - Long-Term Care Re-Certification requires an LPN or LVN license in good standing. Have a copy ready to email or fax.
- Work Experience Reference(s) - Long-Term Care Re-Certification requires candidate to have at least 2000 hours of long-term care experience in the previous three years. Write the name, address, phone number and contact name for employers that can verify work experience.
- Continuing Education - Complete the continuing education document that is part of the re-certification packet included from step one. Be sure you have enough hours listed. Make extra copies of this page if needed.
- Sending Application and Documents - Now either email the certification department or fax us (703) 940-4089 the following:
- Completed Application
- Copy of your license
- Work verfication contact information
- Continuing Education Document
- Email address or fax number where you would like the certification department to send the new certification card.
- New Certification Card - Once the the certification department has verified the application for re-certification a new certification card will be emailed.
